HIPAA Statement In order to comply with implemented privacy provisions of HIPAA, UPNI member pharmacies may need to have a business associate agreement in place with us. As a service to our members, we have created an agreement that you can use for this purpose. We understand that your privacy is important. To better protect your privacy we provide this notice explaining our online information practices and the choices you can make about the way your information is collected and used. To make this notice easy to find, we make it available on every page of our site and at every point where personally identifiable information may be requested. This notice applies to all information collected or submitted on our website. When you simply browse our site, we do not collect any personal information. The only personal information collected by us is when visitors voluntarily email us, request further information via email, or register to have access to our online materials. When you register to have online access to UPNI resources, we collect basic information about you and the pharmacy with which you are affiliated. We use the information you provide about yourself to help us verify your identity, to set up online access to our proprietary materials, and to provide information to existing network members, and pharmacies and pharmacists who are interested in becoming members of our network. We do not share this information with outside parties unless you have signed an agreement and are approved to be part of our network. Then, we only share your information with our contracted clients for which we deliver pharmaceutical care and services and business associates who provide special services that are of benefit to our membership. We use email addresses to answer the email we receive, to send requested information, and, on occasion, to let you know when we add new information, features, services or programs to our site. For member pharmacies and pharmacists, we will, from time to time, use email addresses to send network news, special health plan bulletins and other important contract news. Email addresses are not used for any other purpose and are not shared with outside parties unless you have otherwise agreed to the usage of your email for communications with UPNI contracted parties or business associates. Finally, we never use or share the personally identifiable information provided to us online in ways unrelated to the ones described above without also providing you an opportunity to opt-out or otherwise prohibit such unrelated uses. Our Commitment To Data Security To prevent unauthorized access, maintain data accuracy, and ensure the correct use of information, we have put in place appropriate physical, electronic, and managerial procedures to safeguard and secure the information we collect online. Our site requires users to give us unique identifiers in order to log into the secure areas of our site. These identifiers include the pharmacy NABP number and user-defined password. We utilize these unique identifiers to verify the user's identity and prevent the release of personal information to unauthorized users. UPNI strongly encourages you to take appropriate steps to safeguard your personal password to help protect your privacy.
